Pre-term Labor and Delivery


Pre-term labor : Regular uterine contractions leading to cervical changes (effacement and dilation) before 37 weeks.

Pre-term delivery : Birth that occurs before 37 completed weeks of gestation.

Classification by Gestational Age

Late preterm 34–36 weeks
Moderate preterm 32–34 weeks
Very preterm 28–32 weeks
Extremely preterm <28 weeks
Causes & Risk Factors

History of previous preterm birth

Multiple gestation (e.g., twins, triplets)

Uterine or cervical abnormalities

Infections (e.g., urinary tract or intra-amniotic)

Short cervix (<25mm on ultrasound)

Smoking, substance abuse

Stress, low BMI, poor prenatal care

Signs and Symptoms

Regular uterine contractions (often every 10 minutes or more frequently)

Pelvic pressure

Low back pain

Vaginal spotting or bleeding

Change in vaginal discharge

Cervical changes on exam (dilation and effacement)

Diagnosis

Clinical exam : Cervical dilation >2 cm and effacement

Transvaginal ultrasound : Cervical length <25 mm is concerning
