What Is PMOS?
Polyendocrine Metabolic Ovarian Syndrome (PMOS) β officially renamed from PCOS in May 2026 β is a complex hormonal and metabolic disorder affecting multiple body systems. The new name better reflects the condition's true nature: it is not simply about ovarian cysts, but involves insulin resistance, adrenal and thyroid interactions, and systemic hormonal disruption.
Symptoms of PMOS
- Irregular, infrequent or absent periods
- Difficulty conceiving
- Excess facial or body hair (hirsutism)
- Persistent acne β especially along the jawline
- Hair thinning or loss from the scalp
- Unexplained weight gain, especially around the abdomen
- Dark skin patches in body folds (sign of insulin resistance)
- Fatigue, mood changes, and difficulty concentrating

Investigation & Diagnosis
- Full hormone panel β LH, FSH, AMH, testosterone, prolactin, TSH, DHEAS
- Fasting insulin and glucose tolerance test β to assess insulin resistance
- Lipid profile and liver function tests
- Pelvic ultrasound β antral follicle count and ovarian morphology
- Vitamin D and thyroid evaluation
Treatment Options
- Lifestyle intervention β targeted nutrition, exercise, and sleep optimisation. Even modest weight loss (5β7%) can restore ovulation in many women
- Insulin sensitisers β metformin to address the core metabolic driver
- Hormonal regulation β tailored to whether you are trying to conceive or managing symptoms
- Ovulation induction β letrozole or clomiphene for women trying to conceive
- Skin and hair management β anti-androgen therapies where indicated
- IUI or IVF β if ovulation induction alone is insufficient
Many women with normal or low body weight have significant PMOS and insulin resistance. Weight alone is not a reliable indicator. If you have irregular periods and any other PMOS symptoms β regardless of your weight β you deserve a full evaluation.
