What Is 3D/4D Fetal Ultrasound?

Standard 2D ultrasound provides flat, cross-sectional images of your baby. 3D ultrasound generates three-dimensional surface images β€” allowing visualisation of facial features, limbs, and external anatomy in detail. 4D ultrasound adds the dimension of real-time movement, enabling parents to see their baby's facial expressions and movements as they happen.

Clinical Uses of 3D Ultrasound

  • Facial abnormality assessment β€” cleft lip and palate are better visualised on 3D imaging
  • Limb and hand/foot anomalies β€” finger and toe count, club foot assessment
  • Skeletal dysplasias β€” detailed limb length and bone structure assessment
  • Uterine and placental anatomy β€” fibroid mapping, placental edge assessment
  • Fetal position β€” particularly useful for breach presentation assessment

When Is 3D Scanning Done?

The optimal window for 3D facial imaging is typically between 26–32 weeks β€” when there is enough fluid around the baby and the baby's face is developed but not yet too engaged in the pelvis. Earlier scans can be done for specific indications such as cleft lip screening at 18–22 weeks.

A Note for Parents

3D scanning is a wonderful way to see your baby before birth. However, it is important to understand that a 3D scan is complementary to β€” not a replacement for β€” a standard anomaly scan performed by a skilled sonographer. The anomaly scan (Level II ultrasound) at 18–20 weeks remains the most important fetal structural assessment.
